Online Application Process (Step-by-Step)
1. Go to the Official Beneficiary Portal:
Visit the official National Health Authority (NHA) website:

2. Login as a Beneficiary:

On the homepage, look for the box labeled “Login as” and select “Beneficiary”.

Enter your Mobile Number and click “Verify”.

You will receive an OTP on your mobile. Enter the OTP and the Captcha code.

Click “Login”.
3. Search for your Name:

State: Select your State.
-
Scheme: Select “PMJAY”.

District: Select your District.
-
Search by: Choose “Family ID” (Ration Card Number) or “Aadhaar Number”.

Enter the number and click the Search icon.
4. Identify & Apply (e-KYC):

If your family is listed, you will see a list of names.

If your card status is “Not Generated”, look for the “Action” button (small e-KYC icon) next to your name.

Click it and verify using Aadhaar OTP.

Note: You will receive an OTP on the mobile linked to your Aadhaar.

5. Verification & Photo:

Your Aadhaar details will be displayed. Match them with the system details.

Capture Photo: You must click a live photo using your device’s camera.

Additional Info: Enter your mobile number, pin code, and district again if asked.

Click “Submit”.
6. Download Card:

Once approved (often instant or within 24 hours), the status will change to “Verified” and “Approved”.

Click the Download button to save the PDF of your Ayushman Card.
Offline Application Process
If you cannot apply online or face technical errors, visit:

Common Service Centres (CSC): Visit your nearest Jan Seva Kendra. Carry your Aadhaar and Ration card. The operator will do the biometric KYC (fingerprint) for ₹30.

Empanelled Hospitals: Go to any government or private hospital under the Ayushman scheme. Look for the “Ayushman Mitra” desk. They will register you for free.
How to Check Status/Track Application

Search for your family using Aadhaar/Ration Card.

Verified: Card is ready.

Pending: Waiting for approval (usually by State Health Agency).

Common Problems and Solutions
